City EngineerfUtiliti s irector SUBJECT: Addendum to Previous Proposal- Hydrogeologic Evaluation at Golf Course On March 7, 1995 the City Commission authorized PSI to begin work on a hydrogeologic evaluation and load test program at the Forest Lake golf course and the wastewater treatment plant to determine the capacity of these sites for effluent disposal. This determination was required so that the Florida Department of Environmental Protection could issue a permit to the city for effluent disposal. In discussing this project with FDEP, it was decided that some of the planned deeper auger borings would not be required, but that there was a need for several additional shallow piezometer borings. I authorized PSI to continue with the work in order to expedite determination of the effluent disposal capacity of these two sites, and have been in contact with PSI and FDEP to see if the total number of borings could be further reduced as more information was gathered. The Department of Environmental Protection remains firm in their desire to have data collected from the fifteen shallow piezometers that were not anticipated in PSI's original proposal, but they have also agreed that the scope of the deep borings can be reduced from 210 feet to 100 feet. The total cost to the City for this additional work is $4,350.00 as outlined in the attached Addendum Proposal. This amount will be paid for from the Wastewater Reserve for Contingency account which has a balance of$263,331.50 as of June 13, 1995. I recommend that the City Commission authorize the Mayor and City Clerk to execute Addendum No. 95-2 in the not-to-exceed amount of$4,350.00 in accordance with their proposal number 757-55049 dated April 27, 1995. SUBJECT: Addendum Proposal Hydrogeologic Evaluation/Load Test of City of Ocoee Waste Water Treatment Plant Site and Forest Lake Golf Course, Ocoee, Florida Dear Mr. Shira: This letter is addressing the Plan of Study that has been developed for the load test monitoring and effluent disposal capacity evaluations for the City of Ocoee Wastewater Treatment Plant (WWTP) site and Forest Lakes Golf Course, as discussed in our last meeting with you and PEC. As previously discussed this Plan of Study is based upon our evaluation of the available data, site visits and meeting with the Florida Department of Environmental Protection (FDEP) for their review and approval prior to implementation. Therefore, in accordance with our report entitled "Recommended Plan of Study for City of Ocoee Wastewater Treatment Plant and Forest Lakes Golf Course, Ocoee, Florida" dated April 10, 1995, as accepted and approved by FDEP, it is necessary to add 13 piezometer boring installations, not previously listed in our Addendum No. 95-1. These 13 piezometer borings were approved in our last meeting with PEC and yourself, and are herein presented as an addition to Addendum No. 95-1. Note should be taken that Addendum No. 95-1 shows 210 lineal feet of proposed Auger Borings. However, currently we only require 1 deep boring to 100 feet below existing grade, for groundwater modeling purposes. Please note that we are crediting you with the difference in unused drilling footage, i.e. $715.00 as shown in the following price attachment. Furthermore, as per FDEP's latest request in accordance to their letter OCD-DW-95-0371, dated April 25, 1995, two additional piezometers will need to be installed as a condition of final approval of our "Submitted Plan of Study" discussed previously. The costs incurred for this undertaking are also reflected in our price attachment. The total number of shallow aquifer piezometers required by FDEP is fifteen (15). PSI•1675 Lee Road•Winter Park,FL 32789•Phone 407/645-5560•Fax 407/645-1320 City of Ocoee -2- Project No. 757-55056 Below is a detailed breakdown of all costs that will need to be incurred: • Laying out piezometer location $ 175.00 (Actual survey control and ground elevation to be performed by PEC.) • Fifteen (15) Piezometer Installations @ $14.00/LF $4,200.00 (Assumed 300 Lineal Feet of PVC Pipe with Materials)* • Engineering (layout, meetings, site visits, etc.) 8 Hours @ $65.00/Hour $ 520.00 • CADD Graphics, 4 Hours @ $30.00/Hour $ 120.00 • Technical Secretarial, 2 Hours @ $25.00/Hour $ 50.00 Total: $5,065.00 • Auger Borings (Credit from original scope) (210-100) = 110 Lineal Feet @ $6.50/LF - Credit: < 715.00> TOTAL INCREASE IN BUDGET: $4,350.00 * The necessary and exact footage will be substituted upon completion of the field installation program, which will be reflected in final pricing. PSI will start these piezometer installations on May 1, 1995, as discussed and approved. As also conveyed in FDEP's letter of April 25, 1995, the load test monitoring is to be extended through the rainy season,therefore,the actual monitoring period may exceed the 4± months we recommended. jai City of Ocoee -3- Project No. 757-55056 Should you have any questions concerning these additional services, please do not hesitate to call the undersigned at (407) 645-5560. Project Engineer Senior Vice President Attachment • Addendum No. 95-2 e7i ADDENDUM NO. 95-2 TO AGREEMENT FOR CONSULTING ENGINEERING SERVICES HYDROGEOLOGIC EVALUATION/LOAD TEST FOREST LAKE GOLF COURSE & CITY OF OCOEE WWTP SITE This Addendum No. 95-2 hereinafter called the Addendum as of this day of 1995, shall constitute an Addendum to the Agreement for Professional Geotechnical Engineering &Testing Services between the City of Ocoee, Florida (the CITY) and PSI, 1675 Lee Road, Winter Park, Florida (the CONSULTANT) dated the 27th April 1995, hereinafter referred to as the AGREEMENT. WHEREAS, the CITY and CONSULTANT entered into an AGREEMENT whereby the CITY retained the CONSULTANT to provide professional engineering services as required from time to time, and WHEREAS the CONSULTANT will perform consulting engineering services for the CITY defined. NOW THEREFORE, in consideration of the mutual covenants and promises hereinafter set forth on the part of both parties to be kept and performed, the parties do mutually agree as follows: Section 1 - Services of the Consultant 1.1 The CONSULTANT shall perform the engineering services as outlined in the attached proposal #757-55049 dated April 27, 1995 relative to the provision of consulting services to the CITY. Section 2 - Compensation 2.1 Compensation by CITY for engineering services provided under this Addendum shall be computed under Section II of the AGREEMENT utilizing the Payroll Cost Times a Factor Method of Compensation with the compensation for this additional authorization not to exceed $4,350.00 as outlined in the attached proposal, without further authorization from the CITY. Section 3 - Time of Completion 3.1 The CONSULTANT shall provide the services as outlined in the attached proposal.
