Background Summary:
The Ocoee Fire Department is seeking a formal affiliation Agreement with Lake Technical College
(Lake Tech), which will allow personnel who are attending paramedic school the ability to complete
their ride time on the agency's rescue units and receive credit towards their education.
Issue:
Should the Mayor and City Commission approve and authorize the Mayor to enter into an affiliation
Agreement with Lake Tech to enable personnel to receive credit for paramedic clinical ride time?
Recommendations:
Staff recommends the Mayor and City Commission approve and authorize the Mayor to enter into the
affiliation Agreement with Lake Tech.

AGREEMENT
BETWEEN
LAKE TECHNICAL COLLEGE,INC
And
OCOEE FIRE DEPARTMENT
PARTICIPATING INSTITUTIONS
The participating institutions in this agreement are the LAKE TECHNICAL COLLEGE, dba,
LAKE TECHNICAL COLLEGE hereinafter called "School" and "OCOEE FIRE
DEPARTMENT"hereinafter called the Agency.
II. STATEMENT OF AGREEMENT
This is a mutual agreement between the School and Agency that Agency will accept students
from School for supervised learning experiences in the care of patients in accordance with the
provisions set forth in this agreement.
III. GENERAL PROVISIONS OF THE AGREEMENT
1. The term of this Agreement shall be for 1 year and 4 months, commencing January 1,
2019 and ending December 31, 2020. This Agreement shall be subject to review and
renewal upon expiration of the term of the Agreement. Renewal may be accomplished
by a Letter of Agreement signed by both parties.
2. The education of the student shall be the primary purpose of the training program.
3. School assumes full responsibility for the education of its students.
4. The faculty of School shall be responsible for selecting learning experiences for the students
with the assistance and cooperation of Agency personnel.
5. School agrees to comply with the established policies of Agency and will require each
faculty member and student to become acquainted with all pertinent Agency rules,
regulations, and current policies affecting Agency, its staff and its patients. It will be the
responsibility of the Agency Administrator or designated representative to provide the
School and/or faculty members with the above-mentioned rules,regulations,etc.
6. Agency will assist in the educational process through the cooperation and assistance of
Agency personnel,which is provided to the faculty.
7. A representative from School must interact with Agency and provide date of field internship,
desired ALS unit and time scheduled for each session for each student. Agency reserves the
right,due to staffing dynamics,to cancel scheduled student shifts.

IV. SCHOOL'S RESPONSIBILITY
1. To maintain standards recommended in the Rules and Regulations consistent with the
appropriate accrediting or program approval agency.
2. To employ qualified faculty who shall be responsible for determining philosophy and
objectives,developing the curriculum and maintaining a high quality instructional program.
3. The faculty shall be responsible for:
a. Selecting patient assignments and/or specific Agency experience areas in
cooperation with Agency personnel.
b. Planning for concurrent related instruction (informal and formal classroom) as
needed to meet the objectives of the program. This instruction will be scheduled
during the regular school day, as well as, during the evening or night as
competencies require.
c. Supervision of students.
d. Maintaining individual records of class and clinical instruction, evaluating student
competence.
e. Explaining to Agency personnel the roles of the student within the specific
occupation.
4. To provide to the Agency proof of current licensure of the school.
5. To provide to the Agency proof of current credentials of the faculty participating in the
program.
6. To conduct a criminal records check and a 10 panel drug screen of each student to be
assigned to Agency, excluding any current EMT or Paramedic student currently enrolled in
the program, and to provide the results of such criminal record check and 10 panel drug
screen prior to assigning a student to Agency.
V. THE AGENCY'S RESPONSIBILITY
1. To make available to faculty and students Agency facilities,equipment and clinical services
for planned learning experience included in the program curriculum.
2. To share and assist in the responsibility for the guidance of students particularly in any
restricted area.
3. To include members of the faculty of the School in Agency staff meetings when policies to
be discussed will affect or are related to school programs.
4. To provide faculty and students with emergency medical care in case of illness or accidents
incurred while on duty. Necessary care or service to be charged to individuals receiving
service at current rates.
5. The Agency will not hold the School responsible for the replacement of broken equipment
when such damages are incurred during its normal use.
6. The Agency retains accountability for quality of emergency medical services care.

VI. REQUEST FOR WITHDRAWAL OF STUDENT
Agency has the right to request the school to withdraw any student from its facilities whose
conduct or work with patients or personnel is not,in the opinion of the Administrator of Agency,
in accordance with the acceptable standards of performance.
The School may at any time withdraw a student whose progress,conduct or work does not meet
the standards of School for continuation in the program.
Final action on the student is the responsibility of School.
VII. REQUEST FOR WITHDRAWAL OF FACULTY
Agency has the right to request School to withdraw any faculty member from its facility whose
conduct or work with patients or personnel is not, in the opinion of the Administrator of Agency,
in accordance with acceptable standards of performance. Final action on the faculty member is
the responsibility of the School.
VIII. ADDENDUM
The administration of the Agency and the administration of the School mutually agree that the
special limitations,policies,et al.,attached hereto for EMT-Paramedics and Emergency Medical
Technicians shall be a part of all official copies of this agreement.
IX. DISCONTINUANCE OF AGREEMENT
If either party to this agreement wishes to terminate this agreement, it is understood that at least
ninety days notice shall be given by either participating agency,provided that students currently
enrolled in the program shall be permitted to complete the course.

X. RENEWAL OF AGREEMENT
This agreement shall remain in effect until canceled by either party in accordance with Section
IX. This agreement may be revised at any time by mutual consent of all parties.
XL COPIES OF THIS AGREEMENT
Copies of this agreement shall be placed on file and be available to the following:
1. Florida Bureau of Emergency Medical Services
2. The State Department of Education
3. The Commission on Accreditation of Allied Health Education Programs
IN WITNESS WHEREOF, the parties hereto have made and executed this Agreement on the
respective dates under each signature: "Ocoee Fire Department"through its Representative and the

APPENDIX 1 CONTRACTUAL AGREEMENT FOR PARAMEDIC
City of Ocoee Fire Department
Agency agrees to provide clinical experience for the Paramedic program as follows:
A. Clinical and field internship experiences for the Paramedic include observation,
assistance, and application of basic and advanced life support skills as outlined by the
curriculum as deemed feasible by the Agency department director.
B. Schedule: Students will be assigned for a minimum of 12 hours divided among the
various Rescue Units providing advanced life support,not to exceed one student per unit.
C. The students will be covered by Professional Liability Insurance through the School.

APPENDIX 2 CONTRACTUAL AGREEMENT FOR EMT
City of Ocoee Fire Department
Agency agrees to provide experience for the EMT as follows:
A. Clinical and field internship experiences for the Emergency Technician include
observation, assistance and application of basic life support skills as outlined by the
curriculum and as deemed feasible by the Agency department director.
B. Schedule: Students will be assigned for a minimum of 8 hours divided among the various
Rescue Units.
C. The students will be covered by Professional Liability Insurance through the School.
