This Health ....,,. „i an effort to established a health clinic with consolidated health services for the indigent and needy within the West Orange area. To this end, a Board of Directors is being set up to plan, formulate, seek grants and other government and private funding to build this facility as soon as possible. The site for this clinic has been established on a piece of land near the old Starlite Drive-in at 1150 E. Plant Street, Winter Garden, Florida. While discussing the type of people that would serve on the Board of Directors, it appears that people with a planning and grant background was desirable. To this end, I respectfully request that Janet Shira, Community Relations/Projects be selected because of her talents and experience fits the needs of this organization at this time. Lakeshore Drive Ocoee, FL 34761-2258 August 6, 1998 Mr. Ellis Shapiro: The West Orange area of Orange County consists of many individuals from the surrounding communities who rely on the Orange County Health Department, the Community Health Centers Inc. and Health Central for their primary care medical needs. In order to more effectively meet the healthcare needs of These community residents, both now and in the future, the above mentioned agencies have formed a collaboration —the West Orange Health Alliance. The Alliance plans on establishing a clinic that will consolidate all of the services provided by individual agencies into one multi-service site. At this time, I would like to extend and invitation to your organization to join the Board of Directors by selecting a representative to be part of the West Orange Health Alliance. We are excited about this project and are confident that with your input, the healthcare needs of these residents will be better met. Please take the opportunity to read the attached summary. Attachment Orange County Health Department 604 Courtland Street•Suite 200•Orlando,FL 32804 Attachment West Orange Health Alliance Summary The West Orange area of Orange County consists of communities including Ocoee and Winter Garden. Primary care to the indigent and the working poor in that area is currently provided by the Community Health Centers, Inc.,the Orange County Health Department and the Emergency Room at Health Central. In the West Orange area it is estimated that 8,090 persons live in poverty. Thirty seven percent of those living in poverty are children. The county is growing rapidly and the West Orange Area of Orange County is anticipated to experience a 23% increase in population by the year 2006. With this anticipated growth there will be a greater need to meet the healthcare demands of the population. As previously mentioned, the West Orange Area is served by the Community Health Center (CHC), Orange County Health Department (OCHD) and Health Central. In order to maintain services to the indigent and working poor population in this area now and in the future, it was imperative to plan ahead. A collaboration and consolidation has become necessary to co- locate services under a single administration in one clinic facility. This collaboration and consolidation plan was reached as the end product of a thorough study of the West Orange Area including: the financial background of its residents,the healthcare needs of its residents, current healthcare provisions and projected healthcare needs analysis. This clinic would serve an estimated 10,000 primary care clients per year now being served by the CHC, OCHD and through Health Central's emergency room. Services to be provided are: Phase One — family primary care (including OB/GYN, pediatrics, and general/family practice), WIC, immunizations, pharmacy, dental, lab, x-ray, family planning, STDs, HIV, case management, eligibility social services and education. Phase Two—(with available staff, space and other resources)on-site daycare and wellness. Consolidation would result in increased efficiencies of administration and services, while providing a single location for families of varying income and eligibility levels to access a wider range of services. This project would provide: enhanced continuity of services, single eligibility determination, decreased/eliminated duplication of services, expanded services such as OB to CHC clients and dental to OCHD clients (they would not have to travel to the Central Boulevard site) and improved access for patients. Furthermore,the Alliance addresses the inappropriate use iglrost•emergercy room use and provides greater long term stability. P"'""4.4.'
