This agreement has been in place since December of 1995. Work order #5 is provided to meet the requirements of City Ordinance#95-08, 4-3. B3. which directs the Design Criteria Professional to conduct a peer review of the Design-Build Firms detailed working drawings for compliance with the Design/Performance Criteria established. In the upcoming weeks,the Selection Committee will he conducting contract negotiations with DJ Haycook Construction, Inc., the Design-Build Firm of choice. the contract will include value engineering concepts which could impact the design criteria. A peer review of the Design- Build Contract/Price Proposal is also included in work order#5. I respectfully request your approval to have the Mayor and City Clerk sign the proposed work order giving Gee and Jenson the authority to move forward with this phase of the project. Professional Services to be provided by the Design Criteria Professional: 1. Review one Design Build Contract for compliance with the Design Criteria Package (8/1/97). 2. Review one set of Design Build Firm's signed/ sealed 100% Construction Documents for compliance with the Design Criteria Package (8/1/97). Submittals by the Design Criteria Professional to the City: Provide one submittal of the following: 1. Written comments regarding Design Build Contract. 2. Written comments regarding Design Build Firm's signed/ sealed 100% Construction Documents. Meetings: 1. Attend one Owner/ Design Criteria Professional Meetings. Items to be provided by the City of Ocoee: 1. The City shall appoint a representative who shall, on the City's behalf, attend above stated meetings, issue directives with regard to the Design Criteria Professional's Work, authorize the Work, prepare review comments with regard to the Work and approve the Work of the Design Criteria Professional. The City's representative shall be: Bruce Nordquist Director of Recreation Dept. Name Title 2. The City shall appoint a representative who shall on the City's behalf negotiate and administer the Design Build Contract. The City's representative shall be: Donald Carter Finance Supervisor Name Title 3. The City shall submit to Gee & Jenson, prior to execution, one Design Build Contract. The submittal shall include the Contract with related attachments, the Design Build Firm's CPM Project Schedule and Schedule of Values. 4. The City shall submit to Gee & Jenson, prior to Design Build Firm's application for permits, one set of Design Build Firm's signed/ sealed 100% Construction Documents. Items by Others (Design-Build Firm) • Construction Documents (signed and sealed). • Engineering Calculations (signed and sealed) including, but not limited to, Energy Calculations, Structural Calculations, Drainage Calculations. • Permit Applications: Building Permits, SJRWMD, FDEP, and all site related Permit Applications. • Supplemental Geotechnical Investigations. Attachment"B" (11/11/97) Design Criteria Professional Services Work Order#5- Construction Document Review Fees Worksheet Compensation Basic Design Criteria Professional Services: 60 hrs x $65/ hr = $3,900.00 1. Reprographics Reimbursables: reproducibles, blueprints, photocopies. $ 200.00 (These charges will be billed at cost as they arise. The total amount shown is the amount Not To Be Exceeded unless approved in writing by the City. The following rate schedule represents in-house printing costs. Printing company rates may vary. Photocopies: $0.15 per page (8 1/2 x 11-14) - $0.20 per page (11x17) Reproducibles: $1.50 per S.F. Blueline Prints: $0.40 per S.F. Method of Payment The Design Criteria Professional shall invoice amount due based on the percent of the total work order services actually performed and completed. Invoices shall be submitted to the City once monthly. We understand that the City shall retain 10% of the total work order fee until one hundred percent (100%) of the work order services are actually performed and completed.
