Yout approval for work orders 3a , 3b, 3c, and 3d and compensation for each is needed to continue to move along with the future, and most needed, growth of the Recreation Department . These work orders come under Phase #1 and Phase #1A already approved by the Commission . Execution of the Work Order by City shall serve as authorization of DESIGN CRITERIA PROFESSIONAL to provide for the above project, professional services as set out in the documents which are attached hereto and made a part hereof. ATTACHMENTS: (X) Topographic/Utility Survey DESIGN CRITERIA PROFESSIONAL shall provide said services pursuant to this Work Order, its attachments, exhibits and that certain Agreement dated December 5, 1995 between CITY and DESIGN CRITERIA PROFESSIONAL which is incorporated herein by reference as if it had been set out in its entirety. Whenever the Work Order conflicts with said Agreement, the Agreement shall prevail. TIME FOR COMPLETION: The work authorized by this Work Order shall be completed within sixty (60) days from the date this Work Order is executed by the CITY. COMPENSATION: The CITY shall compensate DESIGN CRITERIA PROFESSIONAL Five Thousand Six Hundred Forty-Five and 00/100 Dollars ($5,645.00) plus Reimbursable Expenses. DESIGN CRITERIA PROFESSIONAL shall perform all work required by this Work Order; but, in no event, shall DESIGN CRITERIA PROFESSIONAL be paid more than the fixed fee and reimbursables set. Mims Rd., Ocoee, FL Work Order#3a - Topographic/Utility Survey Scope of Work Refer to Work Order #2/Schematic Design Submittal (3/10/97), Southeastern Survey & Mapping Corp.'s Specific Purpose Survey of the Existing Wastewater Plant/A.D. Mims Rd. (2/12/97) and Work Order#3/ Design Criteria Site Masterplan Scheme 2b (4/1/97) which will be the subject of this work order. The Design Criteria Site Masterplan indicates 29 acres of improvements to the existing 147 acre site with an estimated improvement allowance of $6.2 million dollars. Phase#1 Improvements include the Recreation Center and Required Parking are estimated at approximately $1.4 million dollars. Professional Services to be provided by the Design Criteria Professional: Topographic/Utility Survey as described in Exhibit "A" - Subconsultants Proposal. Required Submittals by the Design Criteria Professional to the City: • Ten (10) signed/sealed Blueprints of Survey. • One reproducible plan of Survey. Meetings: On-site meetings with City personnel where required. Items to be provided by the City of Ocoee: 1. The City shall appoint a representative who shall on the City's behalf provide free access to the site for the Design Criteria Professional and it's subconsultant, issue directives with regard to the Work, authorize the Work, prepare review comments with regard to the Work and approve the Work. Listing of all Utilities providing Service to or have Easement on this City Owned Property. City Engineer to provide mark-up on an aerial site plan (1:200) with all known data regarding locations/sizesiinvert elevations of existing utilities. 4. The City Engineer to provide mark-up on aerial site plan (1:200) with field verified tree location, species and caliper for the 29 acre improvements area. (These charges will be billed at cost as they arise. The total amount shown is $300.00 the amount Not To Be Exceeded unless approved in writing by the City. The following rate schedule represents inhouse printing costs. Printing company rates may vary. Photocopies: $0.15 per page (8 1/2 x 11-14) $0.20 per page (11x17) Reproducibles: $1.50 per S.F. Blueline Prints: $0.40 per S.F. Method of Payment The Design Criteria Professional shall invoice amount due based on the percent of the total work order services actually performed and completed. Invoices shall be submitted to the City once monthly. SCOPE OF WORK Provide a Topographic and Utility Survey of the area that resides approximately in the southern portion of the project site along A.D. Mims Road in accordance with Chapter 61G17-6 F.A.C. to include the following: A. Locate and delineate existing utility piping, equipment and easements (above and below ground) which cross or impact the site, including but not limited to the following: Rail Road, Power, Telephone, Cable Television, Radio, Water Utilities, Wastewater, Stormwater and Natural Gas Utilities. Locate any existing septic tanks or drain field. Also, delineate development restriction: front, rear & side yard setbacks per Gee & Jenson Site Master plan on the survey. Note: Scope of Work inside fence line of Wastewater Treatment Plant is as follows - Locate and delineate asphalt paved parking lot which adjoins entrance drive. Locate utilities. Locate/delineate existing site grade contours at one foot (vertical) intervals. B. Locate and delineate existing site grade contours at one foot (vertical) intervals. Grade includes all paved areas, unpaved areas, ponds and vegetative areas within the limits of area to be surveyed. Mims Road In Front of the Wasterwater Plant Site, Ocoee, FL Scope of Work Refer to Work Order tt3/ Design Criteria Site Masterplan Scheme 2b (4/1/97) which delineates the Ocoee Park & Recreation Center planned improvements to the existing City owned Wastewater Plant Site, Ocoee, FL. The improvements include a new entrance drive which begins at A.D. Mims Road (Orange County Road) and crosses the existing ACL Railroad Right of Way prior to reaching the property line of the site. The addition of an entrance drive apron and turn lanes at/adjoining the existing A.D. Mims Road as they relate to the new entrance drive on the site will be the subject of this work order. Professional Services to be provided by the Design Criteria Professional: 1. Review the following documents provided by others as they pertain to the development of the entrance apron and turn lanes on A.D. Mims Road: Topographic/ right of way/ utilities survey and geotechnical report. 2. Prepare Design Criteria Package Documents: Site Plan depicting location, approximate dimensions, preliminary cross section and general requirements for turning lanes from east and west bound lanes on A.D. Mims Road and entrance drive apron; Performance oriented specifications in accordance with FDOT (as modified by Orange County), Orange County and City of Ocoee requirements; Summary of permits, utility modification agreements and reviews likely to be required such as St. Johns River Water Management District drainage permits, Rail Crossing Permits/ R.O.W. Modifications Agreement and Florida Department of Environmental Protection Permit for Utilities. 3. Prepare a statement of probable cost and update project schedule. Submittals by the Design Criteria Professional to the City: Provide one submittal of the following: • Design Criteria Package; Twelve (12) sets. • Statement of Probable Cost and updated project schedule. Meetings: 1. The Design Criteria Professional shall conduct one Pre-Application Meeting with each agency determined to have jurisdiction over the project to assist in establishing Design Criteria. The results of these meetings will be provided in the Design Criteria Package for use in the bidding, final design and construction of the project by others (Design/Build Firm). 2. The Design Criteria Professional shall attend two (2) Design Criteria Professional/Owner meetings as deemed appropriate by the Owner. The Owner may elect to contract with the Design Criteria Professional to provide these services under a separate work order. 4. Division 0 & Div. 1 Front-end specifications: Instructions to Bidders, etc. Provide Construction Administration Services: Pre-construciton Meeting, Construction Meetings, Process Pay Requests, Process Change Orders, Review Shop Drawings, Construction Observation, As-Built Drawings, Certifications and Final Close-out. The Owner may elect to contract with the Design Criteria Professional to provide these items and/ or services under a separate work order. Items by Others (Design-Build Firm) • Construction Documents including, but not limited to, Roadway & Drainage Plans, Traffic Control Plan, Erosion Protection Plan, Signing & Marking Plan and Signalization Plan where required. Mims Road on the south side of the Ocoee Recreation Center. At this time, it is our understanding that the improvements will consist of the addition of turn lanes to the north side of the mad. However, we also understand that it may become necessary to widen the road to the south side as well. FIELD EXPLORATION As requested, our field program will consist of conducting 5 auger borings on the north side of the road in the proposed turn lanes and entrance drive and 2 auger borings on the south side. The borings will be conducted to a depth of 10 feet The auger borings will be drilled using a flinch diameter truck-mounted continuous fright auger. Each sample wlllbe tentoved from the auger in the field and then examined and visually classified by our crew chief. LABORATORY TESTING Routine labondory visual dassdemfion will be performed along with specific tests deemed necessary (i.e., sieve analysis, Atte/berg limits and organic contents, etc). A sieve analysis test will be conducted on a sample obtained from one of the borings to provide an estimate of the son permeability. ENGINEERING ANALYSIS AND REPORT Engineering anetysis of ail data obtained will be made to evacuate general subsurface condlons. For air analysis, we will require specific bating conditions for the pavements, in addition,we will provide an estimate of the sal permeability at one of the boring loons. Cur recommendations,together with all data developed during the expiration, win be submitted in a written report upon conclusion of the study. TIME AND COST ESTIMATE Based on our present schedule, we could mobilize our drilling equipment to begin wok within one week after receiving a i hodretlon to proceed. We expect the field exgtluatim vial require 1 to 2 days to complete. Our analysis and report preparation will require an additional 4 to 5 days after completion of the laboratory and field programs, however,verbal reacts can be provided shortly after completion of the field program. The costs associated with the aforementioned tasks are not to exceed 31,150.00 without prior written authorisation. TERMS AND CONDITIONS The total cost is based at the unit prices as aftoen on the attached fee schedule(see Attachment J. This part is subject to the following terms and conditions. (1) access to boring locations is to be readily avdlahle to ors truck-moaned drilling equipment, (2) the proposed number of borings and the terra depths will be adequate,(3)undisturbed samples and consolidation tests on fine grained soils are not budgeted into the total cost, (4)Ardarnan &Associates will not take responsibility for damages to underground services; their locations are to be provided by the diem prior to commencement:of the field work,and(5)this proposed exploration is a relatively shallow exploration and is not intended to be an evaluation for sinkhole potential.
