SUBJECT: Forest Oaks Water Treatment Plant Improvements
Change Order Request No. 1
Attached for your review is Change Order Request No. 1 on the Forest Oaks Water Treatment
Plant improvements Project with Florida Contracting Company, Inc. The Forest Oaks Water
Treatment Plant Improvements Project included the installation of a 1000 Kw emergency
generator with fuel storage tank, new master control center including an upgraded auto transfer
switch, landscaping with irrigation along the north property line, and fencing around the entire
City property at the plant site. This change order request was initiated by City Staff to allow for
improvements to the project for liability issues and vehicular movement.
PROJECT ADDITIONS:
There are two changes proposed to the project. They arc described as follows:
Item A - Construct a Stone Driveway to the Fuel Storage Tank:
The original design did not include any type of paved driveway into the site for access to the fuel
storage tank, staff was attempting to keep costs down and not have to construct any stormwater
retention facilities for a paved road. however, the contractor proposed a drivewa> constructed
with crushed concrete that would be compacted to acceptable standards and holdup for vehicular
traffic such as the fuel trucks. The contractor is using this material to provide a good base under
the concrete slab for the emergency generator, fuel tank and control building. The driveway
would be approximately fifteen feet wide and run into the plant site approximately one hundred
feet and have a thickness of eight inches. The contractor's cost is only for additional material
and labor to install the concrete base driveway he is currently installing the base for the slab.
The material will holdup better than limerock or soil cement, in an unpaved state, and will not
point

require stormwater facilities, since it is considered porous. The cost to perform this additional
work is $1,865.00, with no adjustment in contract time.
Item B - Install Remote Fill Box For Fuel Storage Tank:
The original design for the ten thousand gallon fuel tank included a top mounted fill box which is
allowable by code requirements. I lowever, it has been pointed out that most fuel delivery
companies do not allow their employees to climb up on top of the tank to connect/disconnect the
fuel lines and observe the transfer process and monitor any potential overflows, even though
there are approved ladders and stands. An alternative method is to use a remote fill box/overflow
that is at ground level. Using the remote fill box will greatly reduce the potential liability of the
City from a fall from the tank by the fuel company employee or one of the City's employees
because it will significantly reduce the need to get up on the fuel storage tank. The Public Works
Department had these remote fill boxes installed on their tanks at the direction of their fuel
supplier. 'fhe cost to perform this additional work was $3,720.00, with no adjustment in contract
time.
The attachment is a copy of Change Order No. 1 for this project. Neither of these two items have
been accomplished at this time, they arc awaiting approval. the original construction contract
cost was $492,985.00. This change order increases the contract amount by $5,585.00, to a final
contract amount of$498,570.00. This is an increase of 1.13% of the original construction cost.
The cost of this change order will he paid from the 1997 Utility Bond Issue.
I recommend that the City Commission approve Change Order No. 1 with Florida Contracting
Company, Inc. by increasing the contract amount by $5,585.00, with no increase in contract time,
and authorize the Mayor and City Clerk to execute Change Order No. 1.

I. GENERAL
This change order is necessary to cover changes in the work to be performed under this Contract. The
GENERAL CONDITION,SUPPLEMENTARY CONDITIONS, SPECIFICATIONS and all parts of the
Project Manual listed in Article 1, Definitions, of the GENERAL CONDITIONS apply to and govern all
work under this change order. •
II. REQUIRED CHANGES
Item A: Install crushed concrete in the size of#57 stone in the dimensions of 15' feet wide by 100' long to
constitute an unpaved driveway into the Forest Oaks WTP site along the east properly line to serve as serve as a
driveway access for the emergency generator and fuel storage tank.
Item B: Change the fuel storage tank fill port to from a top mounted port to a remote port at ground level.
III. JUSTIFICATION
Item A: Although the design did not call for the driveway,soil conditions would make access to the fuel storage
tank inaccessible.
Item B: Due to liability issues of climbing on the fuel storage tank and demands from the fuel supplier for the
ground level fill port,the remote fill port is necessary.

contract other than matters expressly provided herein.

Contract Price and Time shown hereon constitute full and complete compensation and satisfaction for all costs and
modifications of performance time incurred by the CONTRACTOR as a result of this Change Order. No other
claim for increased costs of performance or modifications oftime will be granted by the OWNER for the Work

Pursuant to our discussion, we propose the following amendments to the contract be
incorporated as Change Order No. I. There are no schedule impacts associated with either
scope of additional work.
A. Construct a driveway off Hackney Praire Drive to the end of the tank/generator slab
General Contractor (approximately 15 ft. wide x 100 fr. long). Provide.place and compact to rough finish grade,
CGC060389 #57 crushed (recycled) concrete Not to Exceed S1865 00. An engineers report on
this material was forwarded under separate cover. The cost is calculated as follows:
Material $10.45 ton x 45 tons $470.25
Hauliing $11.00 ton x 60 tons S 660.00
Labor S140.00 crew hour x 5 hrs S 700.00
Bond expense $ 34 75
♦ B Provide and install a remote fill for grade level fueling operations. Remote fill will
consist of 3 inch,CS, welded piping from the fill port on top of the tank to grade level and is
housed in a lockable.overspill box with drybreak valve,dust cover and bonom drain per
FDEP requirements. We recommend this change for ease of fueling and liability. The cost
for this is$3,720.00 lump sum.
