(407)877.H420• FAX (407)$77-0192 STAFF REPORT TO: The Honorable Mayor and Board of City Commissioners FROM: Bob Smith, Public Works Direct°° ar- DATE: December 18, 2001 RE: Change Order#2 to janitorial contract with Goldenmark, aka OneSource Facility Service ISSUE: Should the Honorable Mayor and Board of City Commissioners approve change order #2 to the referenced contract? BACKGROUND AND DISCUSSION: OneSource is the City's contractor for facility cleaning. Change order #2 removes the Fire Department administration building from the contract. The Fire Department no longer needs Goldenmark's services. The annual savings is $1,644. RECOMMENDATION: Staff respectfully requests Change Order#2 be approved, retroactive to October 1, 2001. BS:jh Attachment nn Mat THE PRIDE OF WEST ORANGE Change Order No. 2, to the Contract between Goldenmark (aka OneSource Facility service, Inc.) and the City of Ocoee. In consideration of a reduction of$1,644.00 per year, ($137.00 per month), Goldenmark (aka Onesource Facility Service, Inc.), agrees to discontinue janitorial service to the Fire Department located at 125 South Bluford, beginning October 1, 2001. Accepted: The above credit and specifications of this Change Order No. 2, are satisfactory and are hereby accepted. All work to be performed under the same terms and conditions as specified in the Agreement unless before or herein modified. OneSource Facility Service, Inc.
