SUBJECT: Change Order#2—Starke lake South Drainage Well
Attached for your review and action is Change Order#2 for the aforementioned project.
The new changes shown on the attached documents involve changes such as the deletion of four
items, equating to the sum of S8,160.00 and the inclusion of four new items possessing a sum of
S11,480.00. The total amount requested for change order#2 =$3,320.00.
Item #4 on Invoice#1 reflects the installation of a security fence. This item was deleted because
a security fence was not utilized during construction, thereby saving $3,960.00.
Item # 9 references 40 linear foot (LP) of 15-inch RCP. The cost of this item was deleted,
because the decision to utilize an alternate piping material (15-inch HDPE) proved to be more
beneficial. Installing HDPE in lieu of RCP saved approximately $600.00.
Items #10, & #1 l (mitered end section and trash rack) were deleted and then added because of a
change in regards to the end treatment for the inlet pipe. It was determined that a concrete
headwall instead of a mitered end section was a preferred alternative due to the location of
certain control elevations that became apparent during construction. The cost for the new trash
rack decreased by $350.00. There were no added costs for changing the mitered end section to a
headwall. The overall saving on these items was $350.00.
The addition of 36 LE of sheet piling was added to the present list of items to be constructed
because the actual elevation shots during construction revealed that the inlet pipe to the drainage
well would be exposed to the environment. In an attempt to protect the pipe and create an
environment that is aesthetically pleasing, it was decided that the installation of sheet piles along
the portion of exposed pipe would be beneficial for the City. This added item was estimated to
he SR,230.00.

As stated above, all of these items combined would equate to a change order of an additional
$3,320.00, which is an increase of approximately 4.15% of the original contract price.
As you may recall, the negotiated contract for construction services between Henderson Wilder
and the City was $157,030.00. The monies allocated for this project was $250,000.00 (Design
and Construction). The projected expenditures (Design and Construction) to date, which
includes the new changes from the second change order, are $196,690.00. This equates to an
overall saving of$53,310.00 for both design and construction services.
Presently, the southern drainage well is installed and awaiting permission for constructing the
sheet piling. The percent completion for the overall project, both North and South Drainage
Wells is approximately 71.4%.
I recommend that the City Commission authorize the Mayor and City Clerk to execute Change
Order#2 in the amount of$3,320.00.

GENERAL
This change order is necessary to cover changes in the work to be performed under this Contract. The
GENERAL CONDITION. SUPPLEMENTARY CONDITIONS, SPECIFICATIONS and all parts of the
Project Manual listed in Article I,Definitions, of the GENERAL CONDITIONS apply to and govern all
work under this change order.

I. The concrete headwall is more cost effective anti easier to install in-lieu of the proposed mitered end
section.
2. The trash rack is needed to prevent large debris from entering the drainage well.
3. The cost associated with constructing IIDPE is less than that of the RCP.
4. Sheet Piles are necessary in order to protect the exposed inlet pipe.
