Background Summary:
During the June 20, 2023, City Commission meeting, the Utilities Department received approval to hire CROM
Coatings and Restorations, a Division of CROM, LLC (CCR) to repair Tank #1 & Tank #2 at the Forest Oaks
Water Treatment Plant (FOWP). During the repair of Tank #2, CCR found that there were additional repairs
needed and provided Staff with Change Order No. 01, outlining several repair options. Upon review of these
options, Staff determined that the additional crack repairs (Option 3) and the repairs to the surface of the
interior dome (Option 1) were of vital importance. Staff notified the City Manager of the additional expense
and authorized CCR to proceed with those repairs after receiving approval from the City Manager.
The original contract to repair both tanks was at a cost of $205,840.00 and CCR provided a quote of
$24,639.75 for the additional repairs, bringing the total cost of the project to $230,479.75.
Issue:
Provide notification to the Honorable Mayor and City Commissioners of Emergency Repair exceeding the City
Manager's normal purchase authority as provided by Section 21.6 Emergency Purchase, Chapter 21 of the
City Code of Ordinances.

Subject: Change Order Proposal No. 01 — Additional Interior Repairs
CONTRACT - Ocoee, Florida

Dear MR Brownold:
CROM Coatings and Restorations, a Division of CROM, LLC ("CCR"), is submitting this change order proposal for the
work as described below in accordance with all applicable codes and standards including OSHA, AWWA, SSPC, and
NACE standard practices.
1. SERVICES TO BE FURNISHED BY CCR
CCR proposes to furnish all supervision, labor, material, and equipment required to complete the work, except as
noted in Paragraph 3. The services to be furnished by CCR are specifically:
This scope of work is to be performed on the following tank:
• 640,000-Gallon Ground Storage Tank (Forest Oaks Plant, Tank No. 2)

a. Option 1: Additional Interior Dome Surface Repairs (50 square feet)
1) Spot repair up to an additional 50 square feet of the interior dome by abrasive blasting and applying
Tnemec Series 218, followed up by 2 coats of Tnemec Series L140 Pota-Pox Plus at 2.0-10.0 MDFT per
coat, per manufacturer's recommendations.
b. Option 2: Exposed Bolster Feet (175 square feet)
1) Spot repair up to 175 square feet of exposed bolsters located on the interior dome by abrasive blasting to
remove scale and severe rust from exposed metal.
2) Coat exposed metal with Sika Armatec 110 Bonding Epoxy.
3) Apply Tnemec Series 218 surfacing epoxy, followed up by 2 coats of Tnemec Series L140 Pota-Pox Plus at
2.0-10.0 MDFT per coat, per manufacturer's recommendations.
c. Option 3: Crack Repairs (166 linear feet)
1) Mechanically rout the crack up to 166 linear feet removing any lose material from the substrate.
2) Apply Tnemec Series 215 (Modified Polyamine Epoxy) to the prepared crack bringing the repair flush with
adjacent surfaces.
3) Apply 2 coats of Tnemec Series L140 Pota-Pox Plus at 2.0-10.0 MDFT per coat, per manufacturer's
recommendations.
d. Option 4: Interior Dome Surface Repairs and Coatings (4,750 square feet)
1) Abrasive blast up to 4,750 square feet of the interior dome concrete surfaces including 3 foot down the wall
removing rust and corrosion from exposed bolster feet, loosely adhered coatings, and degraded concrete
while providing a surface profile equal to an ICRI CSP 5 or greater as per SSPC-13/NACE 6 standards.
2) Coat exposed metal up to 235 square feet with Sika Armatec 110 Bonding Epoxy.

3) Apply Tnemec Series 218 surfacing epoxy as needed, followed up by 2 coats of Tnemec Series L140 Pota-
Pox Plus at 2.0-10.0 MDFT per coat to the surfaces mentioned above, per manufacturer's recommendations.
2. COMMENCEMENT AND COMPLETION
Upon your execution of this change order, CCR will be prepared to start work immediately after approval of our
submittal information; and will undertake to furnish sufficient labor, materials, and equipment to complete this work
within approximately (2 Crew Weeks) working time thereafter. Acceptance of this change order proposal will
add the aforementioned time to the current contract duration.
3. MATERIALS AND SERVICES FURNISHED BY OTHERS
It is understood that the following services shall be provided by others without expense to CCR:
a. Adequate access to the tank including open storage space for our equipment and materials, conveniently located
near the tank to be repaired.
b. A continuous supply of potable water under 50 psi pressure at the rate of 60 gallons per minute for the use of
the CCR crew within 100 feet of each tank site.
c. A continuous supply of electricity during the period of the cleaning: one 30-AMP, 110-volt service for the
operation of our power tools and accessories, located not more than 100 feet from each tank. Please be sure
that all circuit breakers are ground -fault protected.
d. Complete lock -out and tag -out of the subject tank prior to personnel entering each tank. The Client will be
required to provide all materials for this process. The Client will be responsible for demonstrating to CCR that
the lock -out procedure is complete and the tank is rendered "safe" before CCR will perform air quality testing to
enter each tank.
e. Any permit or other fees from any AHJ as may be required for the work including but not limited to all
connect/disconnect, impact and building/construction fees.
f. All professional engineering services.
4. QUOTATION
We are prepared to carry out this work in accordance with the foregoing for the lump sum price of:
Option 1: Additional Interior Dome Surface Repairs (50 square feet) $ 3,631.50
Option 2: Exposed Bolster Feet (175 square feet) $ 12,710.25
Option 3: Crack Repairs (166 linear feet) $ 21,008.25
Option 1, 2, & 3 Sub Total: $ 37,350.00
Option 4: Interior Dome Surface Repairs and Coatings (4,750 square feet) $ 106,150.00
Option 3 & 4 Sub Total: $ 127,158.25
Please Note:
*The additional interior dome repairs are based on 50 square feet of repair at $72.63/SF.
**The exposed bolster feet repairs are based on 175 square feet of repair at $72.63/SF.
***The interior crack repair is based on 166 linear feet of repair at 126.56/LF.
****If option 4 is selected, option 3 will be required. The pricing for options 1 & 2 is included in the option 4
pricing.
CROM is currently working on site and mobilization fee is not included in the price. If additional
mobilizations are required, a charge of $1,500.00 per mobilization will be added to the final bill. The above
prices are only guaranteed if CROM does not require a demobilization.

Periodic and final payment, including any retention, shall be made within 7 days from the date our work is completed,
or the billing is received, or in accordance with applicable state Prompt Payment law, whichever is earlier, and is to
be received by us in our accounting office at 250 SW 36th Terrace, Gainesville, Florida 32607. All costs for third
party contracts and billing management services, or use of any software, as may be required by the Customer or
Owner, will be added to the Contract Price, by change order. Final payment shall not be held due to delays in testing.
Payment not received by that date will be considered past due and will be subject to a late payment charge of 1'/Z%
per calendar month, or any fraction thereof until received in our office.
If CCR does not receive payment within 7 days after such payment is due, as defined herein, CCR may give notice,
without prejudice to and in addition to any other legal remedies and may stop work until payment of the full amount
owing has been received. The Proposal Amount and Time shall be adjusted by the amount of CCR's reasonable
and verified cost of shutdown, delay, and startup, which shall be effected by an appropriate change order.
Any reasonable legal or other expense necessary for the enforcement of this Proposal or for the collection of monies
due shall be borne by the party at fault. If sales tax is excluded from our price, or on materials contained in our price,
in accordance with project specifications, such exclusion is subject to receipt of adequate tax exemption documents
from you or the project owner prior to commencement of Work to allow for our application of same. If proper tax
exemption documentation is not received or is not adequate to provide exemption, we reserve the right to receive
reimbursement of all sales tax CCR is obligated to pay due to the tardiness, lack of, or inadequate exemption
documentation on behalf of you or the owner.
5. ACCEPTANCE
This change order proposal is offered for your acceptance within 10 days from the change order proposal date. We
reserve the right after that period to amend our pricing to reflect changes in construction schedules, materials, and
labor rate changes. The return to this company of a copy of this change order proposal with your acceptance
endorsed thereon within the time aforesaid will constitute a contract between us. This change order shall be made
a part of the original contract agreement.
