The NOV also indicated that algae buildup had occurred within the basins due to the inoperability of the fountains. Apex mobilized to the site to assess the fountains. Upon arrival, the team triaged the control panel for the Northern fountain to determine power root cause of failure. It was noted the Northern fountain required a new capacitor. The technician ordered the capacitor. The capacitor was received on 5/31 and installed into the existing panel. Once installed, the fountain began operating as designed. The southern fountain control panel was triaged and noted to require a new breaker and transformer. The components were ordered on 2/26 and due to issues with the manufacturer, were not received and installed until 6/12. Once the team installed the new components within the panel, the fountain remained inoperable. The electrical technician performed the required electrical triage and noted there was no power being received at the panel from the electrical closet indicating and issue within the main conduit between the two points. A line locate of the main conduit from the electrical closet to the Southern Fountain panel was completed by others and noted to have been wired into the lighting. The repairs to the southern fountain wiring have been deferred to others. Apex received additional correspondence from the AHJ indicating the northern fountain was inoperable on 6/12. Apex met with Walmart Facilities onsite on 6/17 and oversaw the fountain and electrical assessment completed by Facilities. The FS assessment concluded that the fountain pump and motor are inoperable and require total replacement. Apex proposes to install a new fountain and components in the northern pond to restore operation. Both ponds were treated for algae buildup throughout the duration of the triage to remain compliant for algae. Crews returned 7/10 to install the new North fountain. The fountain was successfully installed and crews treated the pond again due to excessive aquatic vegetation. Crews returned 7/16 to further treat the pond. Deficiency 1: Fountains • Southern Pond: o Perform algae removal within the basin pool level. o Triage all control panel components and power supply from the electrical supply closet. o Procure and install new breaker and transformer within the associated control panel. o Perform startup operations to determine fountain operability. o Perform a line locate of the main subsurface electrical conduit from the pond to the electrical closet (performed by others). • Northern Pond: o Perform algae removal within the basin pool level. o Triage all control panel components and power supply. o Procure and install a new capacitor in the northern panel. o Perform startup operations. o Remove all inoperable fountain components and dispose of offsite. o Procure and install a new fountain and associated components to include: Shp fountain with lighting, associated wiring and timer, motor and pump. o Install new lock on the control panel and APEX stickers. • Dispose of all waste materials offsite. • Stabilize any affected areas.
