Background Summary: The Lift Station #7/S.R. 50 Forcemain Project is nearing completion and the existing forcemain will have three private connections still utilizing the main (see attached General Scope of Work Schematic). The forcemain is to be taken out of service with the widening of State Road 50. It is Staff's recommendation to undertake the design and permitting of the re-routing of the private connections so as to completely take the main out of service more promptly. This is the same forcemain that has failed numerous times and will remain pressurized as long as the main remains in use. CPH Engineers was selected and awarded a contract to provide the design and coordination of the relocation of all City utilities for the State Road 50 Widening Project, which includes the forcemain. Staff is recommending a Change Order to the design work to obtain two additional FDEP permits, two FOOT permits, and create a separate set of biddable plans. When the work is completed, staff will negotiate a third construction change order with GWP to cause the re-routing of the private services and to take the forcemain out of service. Issue: Should the City Commission approve a Change Order to the State Road 50 Widening Project Design Consultant Contract, and direct Staff to negotiate a construction Change Order with the Lift Station #7/S.R. 50 Project Contractor to re-route the private connections and abandon the forcemain? Recommendations: Motion to accept and authorize Mayor to execute Change Order to State Road 50 Forcemain Relocation Project from CPH, Engineers, in the amount of $9,164.25 plus $1,000.00 for permitting fees; and direct Staff to negotiate with GWP, Inc. to reroute the existing service connections to the State Road 50 Sanitary Forcemain and abandon said forcemain; and re-appropriate the $10,164.25 from funds remaining in the Bond Proceeds to project # 14103. Attachments: Proposal from CPH Engineers, Inc., General Scope of Work Schematic. Financial Impact: Funding for the design and subsequent construction will come from Bond Proceeds, and this action shall require $10,164.25 to be programmed to Job # 14103 from the funds available in the Bond Proceeds. In addition, it is estimated that up to an additional $40,000 will be needed for the construction portion of the work. Mims Road Ocoee, Florida 34761 RE: Proposal for Change Order Number:2; State Road 50 Forcemain Service Relocations Dear Mr. Smith: Attached please find our proposal for the referenced services to be known as "State Road 50 Forcemain Service Relocation". CPH proposes to provide professional engineering services for the design of approximately 400 LF of new forcemain to connect with an existing City forcemain and the relocation and reconnection of two existing single service forcemains at or about 10000 to 10125 W. Colonial Drive. This work will also include the reconnection of an existing 4" forcemain service from 9580 W. Colonial Drive to an existing City 6" forcemain. Under this proposal, CPH will prepare the necessary construction documents and permitting services as described herein. The additional work associated with our fees and described herein is necessary to be completed in advance of the work proposed in our Ningal contract under PO#1418998. The work under this scope requires a separate plan set and construction notes to be prepared ahead of the other relocation work including four additional permits (FDEP (two)and FOOT (two)) not included under the original contract which are now required since the sequence of the work under this proposal is different from our original contact. CPH has minimized this scope of services to not include work covered in our original contract. CPH proposes to perform the design and permitting scope of services outlined in this document for the lump sum amount of !9, 164.25 as outlined in the services described below. TASK 1 - Preliminary Engineering The purpose of the preliminary engineering phase is to present project alternatives to the City in a manner that will allow the City to make informed decisions as to how the Project shall proceed. 1. Review recent as-built information on two newly installed forcemains based on City records. 2. Evaluate existing conditions along the proposed pipe installation route by site visitation with City staff. Consider current field conditions and the proposed roadway improvements and/or changes that may impact the Project and recommended location of the proposed pipeline. J:\06000.00\Contract\FM Extension\Revised .4.20.09\SR 50 ForcemainRevised.3.1 O.09.doc 3. This project will use up to 8 locates of the 30 locates posed under the original contract amount. No additional locates are proposed. 4. Prepare 4 sets of 30% plans (24" x 36") showing the preliminary layout of proposed construction alignment of the proposed 6" or 8" (size as determined under this scope) forcemain extension to connect to existing lateral lines from pump stations located at or about 10000 to 10125 W. Colonial Drive. Plans will show reconnection of the 4" FM from 9580 W. Colonial Drive to the City's 6" FM. Submit 4 sets of full size plans for review. As requested by the City, all technical specifications will be included on the plans and a project manual will not be provided under this scope of services. CPH will prepare a bid form in addition to the project plans. 5. Prepare a preliminary opinion of probable construction costs for the project based on the 30% drawings. 6. Meet with City staff to review 30% submittal TASK 2 - Survey, Geotechnical and Environmentallnvesti~ations 1. CPH will use the survey from the FDOT SR 50 roadway plans and we do not anticipate that any additional survey services will be necessary at this time. TASK 3 - Preparation of Construction Documents 1. Prepare design documents and submit to the City at the 100% level; four (4) sets of design drawings (24" x 36") and one (1) opinion of probable construction cost estimate. Meet with the City to review if necessary and discuss the 100% submittal, prepare a written list of City comments, submit to City for verification and subsequently revise the construction documents per the City. TASK 4 - Permittin~ 1. Prepare and submit required Project related permit applications and supporting documentation necessary to obtain required permits for construction and operation of the Project from agencies such as the Florida Department of Environmental Protection (FDEP) and Florida Department of Transportation (FDOT). Two FDEP and one FDOT permit are anticipated. 2. Respond to all requests for additional information from permitting agencies. Pay application fee for the FDEP permit. City will reimburse permit application fees paid by the Engineer. 3. Permit Certification including testing and as-built review. TASK 5 - Public Meetin~ and Construction Notifications No Task 5 services proposed J:\06000.00\Contract\FM Extension\Revised.4.20.09\SR 50 ForcemainRevised.3.1 0.09.doc TASK 6 - Bidding Assistance It is CPH understanding that the City will be requesting a Change Order from a contractor currently working on a City construction project to relocate another portion of forcemain in this area, thus this work will not require the preparation of bidding documents and bidding asstiance. TASK 7 - Construction Administration No Task 7 Services are proposed at this time. If the City needs CPH to provide these services please let us know and we can prepare a separate scope off work. We are providing a detailed cost proposal spreadsheet that outlines the man-hours and sub- consultant costs for the work outlined for this scope of services.
