The Employees and Dependents of each City have benefited through this sharing between the (2) two Cities. Issue: The City of Mount Dora has asked to be included in sharing their facility with the Cities of Ocoee and Apopka. Mount Dora will provide to the Cities of Ocoee and Apopka the same number of hours for appointments as will Ocoee and Apopka to the Employees and Dependents of Mount Dora. Recommendations: It is recommended by Staff to allow Mount Dora to share their facility with Ocoee and Apopka in the same manner as Apopka and Ocoee have for the past few years. Attachments: Attached is a Memorandum of Understanding between the (3) three Cities which has already been approved by Mount Dora and Apopka. The Schedule of hours is also attached. Financial Impact: There is no financial impact. The benefit to the employees of Ocoee will include more hours to access appointments at the Mount Dora and Apopka facilities. This Agreement made effective as of day of , 2012, between the City of Ocoee, Florida, (hereinafter referred to as Ocoee), and the City of Apopka, Florida, (hereinafter referred to as Apopka), and the City of Mount Dora, Florida, (hereinafter referred to as Mount Dora). WITNESSETH: WHEREAS, Florida law permits local governments to enter into Interlocal Agreements and Memorandums of Understanding regarding mutual aid, land use, zoning and planning and other items of mutual concern, and WHEREAS, Ocoee, Apopka and Mount Dora have opened on -site Employee Health Center(s), located within each of their respective Cities, and each provide similar services, including primary care, medications, lab work, x -rays, and occupational services, including pre - employment, annual physicals and other medical services through the same third party administrator Crowne /CareHere, (hereinafter referred to as the Provider), and WHEREAS, Ocoee, Apopka and Mount Dora have employees /dependents /retirees eligible to use their respective health centers that reside in each of the three locales, and WHEREAS, Ocoee, Apopka and Mount Dora desire to make available to those eligible who reside in one of the other locales outside of their City of employment, to be served more conveniently by this less costly alternative to medical services, and WHEREAS, The Provider has demonstrated the ability to arrange for such appointments and track the utilization between which employee /dependent /retiree is using which Cities facility, as is occurring presently between Apopka and Ocoee, and WHEREAS, Apopka, and Ocoee have already experienced the collective benefit of allowing their employees /dependents /retirees to share each other's facility, Mount Dora desires to join that partnership, in order to add to the cost savings for each City when providing services rendered by the Provider, and WHEREAS, Mount Dora desires to share their facility with and as Ocoee and Apopka presently share the services of the Provider, under the following conditions: 1. Only eligible employees /dependents /retirees may use any one of the three health centers by making regular appointments on -line or by calling the appointment phone line. 2. The Cities agree to provide occupational services, such as work related triage and pre - employment physicals, and as those appointments may only occur with coordination between the administrative staff of each City. 3. CareHere shall create a reporting mechanism that indicates which facility was used by the eligibles from each City, on a weekly basis. 4. Although there will not be an invoice or any charges from one City to another based on the utilization, the Provider will create a summary of the utilization between the eligible and which health center those eligible used on a quarterly basis. 5. The initial term of this Agreement shall be for a period of twelve (12) months, beginning the effective date above, and after the parties all approve this Agreement. Any or all of the Cities may terminate this Agreement by providing thirty (30) days' notice, in writing, to the non- canceling party(s). Upon completion of the initial term, this Agreement shall automatically renew for additional one (1) year periods unless thirty (30) days written notice of cancelation is provided to the non - cancelling party(s). Any modifications to this Agreement are not valid or binding unless they are in writing and formally approved by the City of Ocoee, the City of Apopka and the City of Mount Dora.
