Water Main Extension Project - (District 1 — Commissioner Grogan)
Background Summary:
The North Side Reclaimed Water Main Extension project is the extension of the 16 inch reclaimed pipeline
along McCormick and Ocoee - Apopka Roads to provide reuse water to the Forest Brooke, Eagle's Landing,
Westyn Bay, Vineyards, Fountains West, and Ocoee Crown Point developments. The construction contract
was awarded to Hazen Construction LLC at the April 16, 2013, City Commission meeting.
By action of the City Commission at the June 19, 2012 Commission Meeting, CPH, Inc. was authorized to
proceed with preliminary layout, detailed design including survey and geotechnical services, and assistance
during the bidding of the project. No construction phase services from CPH, Inc. were included at that time
because the Engineering and Utilities Departments were planning to perform most of the construction services
in- house. City personnel are performing the daily field inspection of the construction, review and approval of
shop drawings and contractor monthly pay requests, answering questions from the contractor and /or property
owners along the project route, and many other administrative tasks related to the project.
However, there may be times when assistance would be needed from CPH, Inc. The attached proposal details
many of the deliverables required to finalize the permits with Orange County and OOCEA to work within their
rights -of -way and possible functions that may require staff to request interpretation and /or assistance from the
engineer of record. The proposal is based upon the request for time and materials and is not a lump sum
proposal. The proposal for Construction Phase Engineering Services with CPH, Inc. on the North Side
Reclaimed Water Main Extension Project is a not to exceed amount of $5,965.00.
Issue:
Should the City Commission approve the change order for construction services with CPH, Inc. for the North
Service Area Reclaimed Water Main Extension Project?
Recommendations
The Engineering and Utilities Departments recommend that the City Commission approve the change order for
construction services with CPH, Inc. for the North Service Area Reclaimed Water Main Extension Project and
authorize staff to amend the current purchase order to CPH, Inc. on this project in the amount of $5,965.00.

RE: Proposal for North Side Reclaimed Water Main Extensions - Change Order
No. 1 - Construction Assistance

Dear Mr. Wheeler:
Attached please find our proposal for the Construction Assistance Services for the North Side
Reclaimed Water Main Extensions. CPH has completed the design and bidding phase
services for the first phase of the subject extensions intended to connect those neighborhoods
within the City of Ocoee reclaimed water service area to the City's reclaim distribution system.
This will allow the neighborhoods to be irrigated with reclaimed water rather than with potable
water.
The City has indicated that the majority of the Construction Administration Services will be
performed by City personnel. CPH proposes our scope of services to consist of:
Providing a sketch and description for the easement across the City of Winter
Garden wastewater disposal site.
2. Providing the City seven (7) signed and sealed sets of the "Conformed"
construction drawings for the City's use during the construction phase of the
Project.
3_ Providing the City with a CD with AutoCAD files of the conformed construction
drawings for the contractor's use in laying out the project and preparing the as-
bulits.
4. Attending the pre- construction conference.
5. Providing clarification and interpretation of the Contract Documents when
requested by the City.
6. Performing any necessary site visits to provide clarification and input in the event
of unforeseen underground conditions.
7. The ENGINEER will assist the City in the review of Contractor provided as -built
drawings /surveys and other as -built data for installed facilities and present any
apparent discrepancies between the as -built conditions and the design conditions
to the attention of the City. Coordinate with the Contractor regarding provision of
the construction as -built drawings prior to final on -site inspections and punch list
preparation.

8. The ENGINEER will prepare a final set of Record Drawings for the City. The
Record Drawing Package will include both hard copies of the plans as well as
electronic files for the project. Deliverables shall include two 24" x 36" drawings
and a compact disk with the project cad files (2012 AutoCad) and PDF files of all
sheets.
Compensation paid to Engineer for services described herein and rendered by principals and
employees assigned to the project are computed by multiplying Standard Classification Billing
Rates as per our continuing services agreement for all classifications of employees directly
engaged on the project, times the number of hours worked on the project.
The Estimated Engineering not to exceed fee for the services described herein is $5,965.00.
The Estimated fee includes $5,015.00 in labor expenses, $750.00 in subconsultant fees and
out -of- pocket expenses of $200.00. The expenses will be specifically identified in the periodic
invoicing, and include such items as transportation and subsistence of principals and employees
when traveling in connection with the project, tolls, telephone calls, photocopies, blueprints and
plots, etc. Invoices will be submitted periodically, indicating the hours expended during that
billing cycle. Progress invoices will be submitted based on the time hours worked by CPH
employees at the time of invoicing. Expenses for items not specifically valued herein are to be
reimbursed to the engineer at the actual cost thereof.
If this Contract is terminated during prosecution of the services prior to completion of the
services, payments to be made on account of that and all prior work under this Contract shall be
due and payable, and shall constitute total payment for services rendered. In addition, upon
termination, engineer shall be paid for any Supplementary Services authorized and rendered.
If you are in agreement with the terms and fees proposed, please sign on the space provided
below and return one (1) copy to us, or provide us with the purchase order for CPH to begin
work on the project. CPH Engineers, Inc. appreciates this opportunity to provide our services to
