To recap the work concluded, the following description is submitted: 1. Foundation/Floor Leveling 2. Steps 3. Rear Porch Carpentry 4. Front Porch Carpentry 5. Second Floor Porch Carpentry 6. Siding and Trim Carpentry 7. Paint Stripping 8. Painting 9. Window/Door Repair 10. Change Order #1 (additional damage found during leveling and carpentry) A. Repair and/or addition of a total of seven foundation piers. B. Repair of beam under north end of house. Cohn 11. Change Order #2 (primarily based upon additional damage uncovered during initial work or funding availability) A. Repair/replacement of siding and trim on second floor rear porch. B. Stripping/painting wood on second floor. C. Construction/installation of wood lattice between all brick piers. D. Replacement of rotten front porch roof beam. E. Replacement of roofing and sheathing on first and second floor front porches. 12. Change Order #3 A. Removal of existing rear porch steps and construction of new steps matching original. B. Painting all porch rafters a second color to match porch floor. As shown in attachment, punch list was short. All work will be completed by March 5, 1993. Staff and the Architect both recommend acceptance of all work and request that the Certificate of Substantial Completion be issued. Action Requested The Mayor and Board of City Commissioners accept work as performed and completed by Mitchell Building Contractors, Inc. and authorize the Mayor to execute Certificate of Substantial Completion. MEB:fdg CERTIFICATE OF SUBSTANTIAL COMPLETION PROJECT: Withers/Maguire House Restoration ARCHITECT: Chalmers Yeilding ARCHITECT'S PROJECT NUMBER: 9104 OWNER: City of Ocoee CONTRACTOR: Mitchell Building Contractors, Inc. • CONTRACT FOR: Renovation of the Withers/Maguire House Phase 1 CONTRACT DATE: October 6, 1992 DATE OF ISSUANCE: March 9, 1993 The work performed under this Contract has been reviewed and found to be substantially complete. The Date of Substantial Completion of the Project or portion thereof designated above is hereby established as March 1, 1993. which is also the date of commencement of applicable warranties required by the Contract Documents,except as dated below. DEFINITION OF DATE OF SUBSTANTIAL COMPLETION The Date of Substantial Completion of the Work or designated portion thereof is the Date certified by the Architect when construction is sufficiently complete, in accordance with the Contract Documents, so the Owner can occupy or utilize the Work or designated portion thereof for the use for which it was intended as expressed in the Documents. The list of items to be completed or corrected, prepared by the Contractor and verified or amended by the Architect, is attached hereto. The failure to include any items on such list does not alter the responsibility of the Contractor to complete all Work in accordance with the Contract Documents. The date of commencement of warranties for items on the attached list will be the date of final payment unless otherwise agreed to in writing. Chalmers Yeilding ARCHITECT BY ATE The Contractor will complete or correct the Work on the list of items attached hereto within thirty days from the above Date of Substantial Completion. Tampa Avenue Orlando, Florida 32805 FROM: Chalmers Yeilding SU BJ ECT: Final Punch List The following is a list of work items to be completed prior to Final Completion: 1. Paint concrete treads and risers to match porch floor. 2. Paint concrete caps at steps white. 3. Sand out visible column patch joints and repaint. 4. Paint P. T. nailer strips supporting lattice flat black. 5. Fill, sand and paint roof gable trusses to remove visible damaged areas on South and West Elevations. 6. Touch up paint in miscellaneous areas. 7. Rake immediate area of house and remove all debris. 8. Remove Port-a-let and job trailer from site. 9. Paint porch rafters to match the floor. 10.Remove rear porch steps and replace with new steps to match original. 11 .Remove excess paint from top edge of second floor front porch beam. 12.Install temporary wood rail across the rear porch door wjthout steps.
