advertised in the West Orange Times, on the City's website, on the OGTV bulletin board, and
postcards were mailed to previous applicants and those organizations who have requested to be
notified of potential grant opportunities. The City received 12 applications (attached).
Issue:
City Commission approval of the recommendations made by the Community Merit Awards Board.
Recommendations
The Community Merit Awards Board respectfully recommends funding the applications as noted on
the attached staff report for a total of $5,000 this cycle. This will/eave $5,000 for the second cycle
which takes place in April.

MINUTES OF THE CITY OF OCOEE
COMMUNITY MERIT AWARD REVIEW BOARD MEETING
HELD JANUARY 6, 2005
CALL TO ORDER
Chairman Sills called the regular meeting of the Community Merit Award Review Board
to order at 7:35 p.m. in the Commission Chambers at City Hall. The roll was called and
a quorum declared present.
PRESENT: Chairman Sills, Members Hood, and Miller. Also, present were Assistant to
the City Manager Shira and Deputy City Clerk Brasher.
ABSENT: Members Axtell and Jacobs, who were unexcused.
APPROVAL OF MINUTES - May 26, 2004
Member Hood, seconded by Member Miller. moved to approve the Minutes of the
Community Merit Awards Board Meetinq held May 26.2004. Motion carried 3-0.
DISCUSSION OF APPLICATIONS AND FORMAL RECOMMENDATION TO THE
CITY COMMISSION
Assistant to the City Manager Shira stated there were 12 application requests with a
total dollar amount in requests of $6,000 and with a total of $10,000 to disperse for this
cycle as well as the Spring 2005 cycle. Community Merit Award Review Board
recommendations are tentatively scheduled to go before the Ocoee City Commission on
February 1, 2005. Ms. Shira further stated that a summary sheet is attached so the
board members know what organizations have received funds in the past.

Chairman Sills announced that no one was present to represent the organization.
Member Miller. seconded by Chairman Sills. moved to recommend to the Ocoee City
Commission approval of $250 to the American Leqion Post NO.1 09. Motion carried 3-
O.
Application No.2 - Central Florida YMCA - Safe Start - A Survival Swim Program
UIi Cordell & Danielle Guarneri were present to represent the organization.
Member Hood. seconded by Member Miller. moved to recommend to the Ocoee City
Commission approval of $500 to the Central Florida YMCA - Safe Start proqram.
Motion carried 3-0.

Motion carried 3-0.
Assistant to the City Manager Shira told the applicants that the board
recommendations would be passed on to the City Commission and the City
Commission would make the final decision on funding for the organizations.

Fact Sheet
. The Blue Star Service Banner was designed and patented in 1917 by World War I Army Capt. Robert
L. Queissner of the 5th Ohio Infantry who had two sons serving on the front line. It quickly became the
unofficial symbol of a child in the service.
. On Sept. 24, 1917, an Ohio congressman read the following into the Congressional Record: "... The
mayor of Cleveland, the Chamber of Commerce and the governor of Ohio have adopted this service
flag. The world should know of those who give so much for liberty. The dearest thing in all the world to
a father and mother - their children,"
. During World War II, the Department of War issued specifications on the manufacture of the flag as
well as guidelines indicating when and by whom the Service flag could be flown or the Service Lapel
button could be worn. The Department of Defense updated the guidelines on December 1, 1967 with
DoD Directive 1348.1, which implemented an Act of Congress authorizing a service flag and a service
lapel button (D.S.C. 179-182).
. The Blue Star Service Banner typically displayed in windows is an 8.5 by 14-inch white field with a
blue stares) sewn onto a red banner. The size may vary but should be in proportion to the size of the U.S.
Flag.
. Today Blue Star Service Banners are displayed by families who have a loved one serving in the armed
forces including the National Guard and Reserves of all military departments (https://www-
perscom.army,mil/tagd/tioh/F AQ/ServiceFlagFactSheet.htm), The banner displayed in the front window
of a home shows a family's pride in their loved one serving in the military, and reminds others that
preserving America's freedom demands much.
. The blue star represents one family member serving in the armed forces. A banner can have up to five
stars, signifying that five members of that family are currently in military uniform on active duty,
. If the individual symbolized is killed or dies while serving the star representing that individual will
have superimposed on it a gold star of smaller size so that the blue forms a border. On flags displaying
multiple stars, including gold stars, when the flags are suspended as against a wall, the gold stares) will
be to the right of, or above the blue stares) a place of honor nearest the staff.
. Blue Star Mothers and Gold Star Mothers organizations were established during World War I and
remain active today.

embraced during the Korean or Vietnam wars with nearly the same enthusiasm.

THE AMERICAN LEGION
SCHOOL AWARD MEDAL PROGRAM
QUALITIES TO BE DEVELOPED
Courage - Bravery in the face of opposition and danger; determination and force to
do right without public applause and regardless of personal advantage: a quality of
the intellect.
Honor - Highly-developed moral character; moral excellence; strength and stability
of character; high standards of conduct; devotion to duty; adherence to truth; keen
sense of what is right; practice of clean speech and thoughts.
Leadership - Ability to lead and to accomplish through group action; ability to work
in harmony and in unison with other leaders in accomplishing group results; desire
and ability to fill the voids in the lives of others caused by timidity, illness and other
handicaps.
Patriotism - An ideal of loyal Americanism, religious tolerance, righteous freedom
and the willingness to defend our Flag against all enemies - foreign and domestic.
Scholarship - Attainments in school studies; quality of schoolwork reflecting the fine
traits of industry; perseverance; efficiency and intelligence,
Service - Kindliness; unselfishness; fellowship; protection of the weak; promotion of
the interests and the welfare of associates and constructive aid for the support of
schools and community.
AWARDS
This medal is the highest honor awarded American youth by The American Legion. It
is a medal that proclaims to the world that this boy or girl is the kind of young
person we believe worthy of such an honor.
The recipients (one boy and one girl) are selected from the graduating class of
elementary, junior high or middle schools, senior high schools and college. In schools
where there are many graduates or where students are grouped (such as sixth grade
centers or special interest schools), special arrangements may be made to cover the
situation. This is up to the discretion of the Post Committee in conjunction with the
school authorities.
SELECTING THE RECIPIENT
The Post Commander will appoint a committee of not more than three members to
contact local school authorities. Through them a basis of friendly cooperation with
school officials should be established. In this way it will be possible to design and
activate a smooth, coordinated program.
Arrangements between school and Post on how the recipients are to be selected
should be made well in advance of the presentation date. The aforementioned
character traits should be the basis of selection. The sole purpose of this program by
The American Legion is the cultivation of high character and wholesome ideals in
youth approaching active citizenship. No trace of politics, sectarianism or favoritism
should be allowed to attach itself to the School Award Program.
A member of The American Legion should present the award. However, all plans
should be made with the cooperation and consent of school officials.
OBJECTIVES
The American Legion School Medal Award Program is designed to: Place emphasis
on the development of the qualities of COURAGE, HONOR, LEADERSHIP,
PATRIOTISM, SCHOLARSHIP and SERVICE.
Help develop those ideals of Americanism among young people, which will make
them citizens of the highest caliber.
Give recognition through the award of American Legion School Medals to the boy
and to the girl who have demonstrated the highest qualities of citizenship and of
true Americanism.

granted Safe Start an award of $500.00. Thank you,
Does your organization volunteer in the Ocoee community? If yes, what volunteer
activities do you participate in? Safe Start Instructors participate in various public and
private health and safety related events and activities. (ie Health Fairs, Church events, etc)
How many yearly volunteer hours does your organization contribute to the community
and how are those hours tracked or documented? 40 + volunteer hours contributed
annually and are documented on a volunteer justification form.
Please list the specific accomplishments your organization has achieved in the past
24 months: In the past 24 months, Safe Start has taught the Survival swim skills necessary
to survive an aquatic emergency to 2100 children in Central Florida. Not one of these
children has become a victim of drowning or near drowning. 19 of these children have used
their skills to survive an unsupervised, accidental fall into the water. No one is turned away
by the inability to pay course fees. Each year Safe Start gives $25,000 + in financial
assistance to families who can not afford to pay the normal course fees.
Please include any other pertinent information about your organization or this specific
grant request that you feel would help the City in making its decision: Safe Start is a
proactive drowning prevention campaign which teaches the survival skills of
swimming and floating to children ages 12 months to 4 yrs. Old. The Safe Start Program is
offered at the West Orange Roper YMCA and is taught there by Ocoee resident, UIi Cordell.

This letter is written on behalf of the Safe Start survival-swimming program at the
Central Florida YMCA. This shall serve as verification that I am in support of the
grant application, as prepared by Mrs. Lili Cordell, Safe Start instructor. This
request for financial assistance is in the amount of $500 from the City of Ocoee.
If granted, the funds received will go directly to the Central Florida YMCA on
behalf of the Safe Start scholarship fund and will allow families to participate in
Safe Start lessons who are unable to afford the normal course fees.

CONSTITUTION
ARTICLE I - NAME
The name of this organization shall be WOMAN"S CLUB OF OCOEE, INC.
ARTICLE 2 - PURPOSE
The OBJECT of the Woman's Club ofOcoee, Inc. shall be civic, charitable,
educational, literary, scientific, and generally to give aid to all worthy projects.
Upon DISSOLUTION, no part of this organization's earnings shall inure to the
use 'or benefit of any individual. In the event this organization should be
dissolved for any reason, any remaining assets shall be ~istributed for purposes
within the scope of the OBJECT stated above.
ARTICLE 3 - MEMBERSHIP
Section 1. The membership shall consist of three classes: Active, Associate,
And Honorary.
Section 2. The active members shall take part in all activities of the Club,
Including literary and committee work assigned to them. Only
active members shall have part in the property holdings.
ARTICLE 4 - OFFICERS
The officers of this Club shall be: President, First Vice-President, Second Vice-
President, Recording Secretary, Corresponding Secretary, and Treasurer. These
, Officers shall be elected at the May business meeting for a term of one (1) year.
No officer shall be eligible to serve more than two (2) successive terms.
ARTICLE 5 - EXECUTIVE BOARD
The Executive Board shall consist of ten (10) members: President, First and
Second Vice-Presidents, Recording and Corresponding Secretaries, Treasurer,
Immediate Past President, and three (3) additional members elected by the Club.
The Immediate Past President automatic8Ily serves in that capacity for one year,
directly following her presidency, and may serve a second consecutive term, if
duly elected.

BUSINESS MEETING
TUESDAY, OCTOBER 12, 2004
Myra called the meeting to order at 7:01 p.m. There were 26 members present.
Myra read the minutes from the previous business meeting.
The board recommended that a donation of $1 000.00 be made to Rick Osborne for his
labor on the clubhouse windows. It was seconded by MaryJane Antonov and the
recommendation passed.
The board recommended that in lieu of a gift exchange at the Christmas meeting that all
members would be asked to donate $5.00 to Miles for Smiles wish list. Elizabeth
Maguire seconded it and the recommendation passed.
The board made a recommendation that Renee Epps will start a website for the club at the
cost of $500.00. Judie Lewis seconded it and the recommendation passed.
Renee Epps asked that any information for the newsletter be given to her by Monday
October 18.
Nancy Maguire led ~ in devotions and said the prayer after several prayer requests were
made.
Nancy Maguire read the Treasurer's report.
The board recommended that we purchase 41arge banquet tables from Sam's Club at a
cost of 69.86 per table. Judie Lewis seconded it and the recommendation passed.
The board recommended that Diane Carter will look into having the sides of the stage be
made into storage. She will talk to a carpenter and get estimates and we will discuss this
at a future board meeting. Nancy Maguire seconded it and the recommendation passed.
"Diane Carter proposed that a Juniorette club be looked into for our area for girls from age
12 to 18. Renee Epps seconded the motion and the motion passed.
The board recommended that we allow the Catholic Church to rent the clubhouse on
Sunday mornings for a period of about 6 months at a deposit of $250.00 and $150.00 per
week. Cheryl Applegate seconded it and the recommendation passed.
Diane Carter proposed that we do the Gift of Giving night at the mall again this year as a
fundraiser. Vicki Raduenz seconded the motion and the motion passed.
The tickets for the Birthday Luncheon will be available at the November business
meeting.
Ellen King will be setting up the Art Show for the Founder's Day Celebration on
Saturday, October 16. A motion ~ made by Elizabeth Maguire for her to purchase
ribbons at 27 cents each. Nancy Maguire seconded it and the motion passed.
Also, Renee Epps will make certificates for all the students.
The board made a motion to donate $200.00 to an Orlando Family in the Miles for Smiles
program.. Renee Epps seconded it and the motion passed.
The meeting adjourned at 8:15 p.m.

The minutes were reviewed and approved.
If your school PTA is not involved or aware of S.T.A.R.S., be sure to tell them about us.
Many PTA's mention us at meetings or place information on their websites to make parents aware
of our organization.
Our scissor committee met and reviewed letters to send out from each school. These
letters will be finalized and sent, via the school courier, to all schools associated with S.T.A.R.S.
Each school can make copies and send out when appropriate. One letter is to go out right away
and the other letters to teachers and parents are reminders, and can go out toward the end of the
school year. We received a donation of wood to help us get started with this project. Chesta will
try and get in touch with some carpentry classes to see if they will make holders or donate wood.
The weather finally cooperated and our Chevys fund raiser finally happened. We raised
about $70.00. The committee thought that maybe for our next one, we should try another place
that would be a little less expensive. Chesta will check with Pizza Hut to see what kind of fund
raisers that they offer.
The West Oaks Mall will be hosting a "Festival of Giving" on Sunday, December 5th. The
mall will be open to ticket holders only from 6:30 pm to 9:30 pm. Tickets can be sold for $5.00 and
S.T.A.R.S. can keep 100% of the money collected for the sell of our tickets. The committee
decided to try and sell 25 tickets at each school and see how we do. We will also have a table
display in the mall for participants to learn more about us, This table will need to be manned by a
S.T.A.R.S. representative. We will need to take turns doing this throughout the evening.
The Florida Auto Auction will let us use a parking lot and will assist us with tables for our
Community Garage Sale. However, after discussing it at the S.T.A.R.S. meeting, a suggestion
was made to change the location to St. Pauls parking lot. Another suggestion that came up was
for each school to hold their own sale on their school campus. That way it would not be such a big
burden for a few people to take on. This will need to be talked about at our next meeting.
Fabric has been purchased for our quilt and we have a volunteer to help cut out squares.
YEAH!
Disney Hand: Helping Kids Shine Awards are coming up. The deadline is November 19,
so we need to get the information to them.
City of Ocoee, Community Merit Award is a grant for $500.00. The committee decided
that we should apply for this grant during the November, 2004 Cycle.
Chesta showed us a plaque that we received from Maxey for being a Partner in
Education. When we get a office, we can display it on our wall. Any school who wants to use
S.T.A.R.S. as a Partner can do so.
Louis shared about "Relay for Life" that will be held at West Orange High School on April
8th & 9th. It will begin at 6:00 pm. There is a $100.00 entry fee. Health Central will begin hosting
Team meetings starting in January. If you are interested in helping or participating in any way call

Backeround:
On 13 May 1958, the Internal Revenue Service declared that Rotary International (Rl) and its clubs and districts came
under Section 501 (c)(4) of the Internal Revenue Code and that status is still in effect. While Rotary districts and clubs
are exempt from payingfederal income tax, the law does require Form 990 to be filed on or before the 15th day of the
5th month following the close of the annual accounting period (15 November). -- An administrative ruling waives that
requirement for those with average annual revenues of less than $25,000.
Numbers: Rl's Group Exemption Number (liGEN" on Form 990), is 0573, which applies to all clubs and districts. At
the same tim,e, every club and district in the US is required to have its own Employer Identification Number (ErN).
That 9-digit number is really a tax ill number and has nothing to do with whether there are any employees. As R1
must report annually the EINs of all districts and clubs in the US, the secretariat maintains a list of those tax numbers
and also applies for numbers to be assigned to new Rotary clubs. You will be notified directly by the IRS of your
club's EIN number. -
Tax Deductible Status: Though clubs and districts do not have to pay federal income tax, donations to clubs and
districts are not tax deductible. Such status is, however, possible for a contribution to any club or district charitable
fund that has been independently classified by the IRS as being under Section 501(c)(3) of the Internal Revenue Code.
As to membership dues, they may be deductible, for some Rotarians, as a business expense.
Other Taxes: Clubs may be required to file Form 990- T if they have "unrelated business income." A club or district
which has gross receipts of $1,000 or more during a year from an unrelated trade or business must file a Form 990- T.
An unrelated trade or business is defmed as: "Any trade or business that is regularly carried on, and that is not
substantially related to the organization's exempt purpose or function."
Social security and withholding taxes may be required from a club that has one or more paid employees,
There may be a tax or filing obligation in regard to prizes and awards (including scholarships),
The laws of the states vary in regard to state income tax, sales tax, other taxes and speCial licenses. The 501(c)(4)
status applies only to federal income tax and does not necessarily have any bearing on other taxes. IRS interpretation
of its Code also can vary slightly in different Treasury districts.

increased from 12 to 110 from August of 2002-August of 2004 We had the first AVID
graduate from OCPS last May, now attending UF,and 11 seniors will graduate this May!

The purpose of. AVID is to give average students the opportunity to go to college
by enrolling them in rigorous honors and Advanced placement courses and the AVID

We discussed both literacy breakfasts. On November 9th our breakfast will be about the
process of writing from beginning to end beginning with concept vocabulary
development, moving on to the planning stage and ending with the finished written
product. The writing prompt will be the walkathon and the nursing program.
. Each grade level with give a short presentation of what the process looks like at
the specific grade level (including work samples)
. We will present in grade level order to demonstrate the scaffolding of skills from
one grade level to another.
. Responsibilities were assigned:
1. Isabel will make food arrangements with Mrs. Stepp.
2. Dian will make stickers and make arrangements with Tammy for A V equipment.
3. Each grade level literacy member will present grade level information
4, Richard will put together parent packets (given to him by grade level presenters).
Each grade level is in the process of developing a student rubric to assist students with
writing goals. The purpose of the January breakfast will be to explain rubrics to parents,
. Each grade level will give a sort presentation of grade level specific rubrics
. We will present in grade level order to demonstrate the scaffolding of skills from
one grade level to another.
. Grades 3-4 will have the same rubric, so teachers will further demonstrate how
the rubric score is used to develop learning goals for future writing.
Isabel Chipungu requested fimds to provide the media with DVD's, Mrs. Sheilla Johnson
reported that no n.mds were available to purchase those materials and suggested Mrs,
Hooper write a grant request to the City of Ocoee to provide the Literacy Committee with
$500,00 to make that purchase

Food Drives, School and church presentations

social skills training and summer camp enrichment opportunities, As stated above we are
moving towards having meals avajJabJe 7 days per week early in 2005. Through varmus
collaborations our agency has assisted countless individuals and families, This past
Thanksgiving, with the help of the City of Ocoee employees, we distributed 43 Holiday
Baskets to case managed clients, This partnership allows us to reach more deserving
families in need. Our partnership with St. Luke's Methodist Church made it possible to reach
125 additional families with Holiday Baskets.

through Student Services for future scholarship opportunities.

24 months: Hosted State Renaissance conference in 2003, received the 2003 Spirit Award
and the 2004 VTW (Visible, Tangible, Walkaroundable) Award at the National Renaissance
Conference, presenters at the 2003 and 2004 National Conferences.

grant request that you feel would help the City in making its decision: Renaissance is a
program that recognizes the academic achievement and improvement of students through
celebrations. By putting the focus back on academics, we have seen a renewed enthusiam
for learning and success from our students and teachers. Students who have been
recognized through Renaissance feel a connection to West Orange High School and know
that what they are accomplishing in the classroom is important to us.
