Subject: Interlocal Agreement Between City of Ocoee and Belle Isle with Crowne /CareHere
Background Summary:
The City of Belle Isle, Florida (City Manager) has requested of the City of Ocoee (City Manager) to consider
partnering with Belle Isle to provide limited occupational services, i.e. pre - employment and annual physicals for
their police officers. Belle Isle has approximately 15 officers and hires about 3 officers per year. Belle Isle is
having difficulty finding a provider that can service their occupational needs for comprehensive physicals.
Issue:
The City of Ocoee desires to provide Belle Isle limited occupational services through the CareHere health
center for pre - employment and annual physicals. This partnership is estimated to consume approximately five
(5) regular appointments per month at the health center. The small number of appointments to be used will not
have an adverse impact on the City of Ocoee employees /dependents.
Recommendations
It is the recommendation of staff that the City Commission approve the attached Interlocal Agreement, whereby
the City of Ocoee through the health center will provide limited occupational services to the City of Belle Isle.
The agreement shall be administered and coordinated by the Crowne Group. The invoicing and administrative
charges to Belle Isle and credits back to the City of Ocoee shall be managed by the Crowne Group.
Attachments:
Attached is the Interlocal Agreement and Appendix A. The appendix lists the type of services to be provided
and the associated costs to be paid by Belle Isle for each type of physical and ancillary costs if requested. This
agreement may be terminated by either City with a thirty (30) day notice.
Financial Impact:
There will not be a financial impact to the City of Ocoee for Belle Isle will be invoiced and pay the staff time for
the number of appointments, all the costs of lab work and any other type of test performed through the health
center on a monthly basis. There will not be an increase in City of Ocoee staff time. The City of Ocoee shall be
credited for the cost of staff time (appointments) and the associated costs to the health center.

INTERLOCAL AGREEMENT
BETWEEN
CITY OF OCOEE, FLORIDA
AND
BELLE ISLE, FLORIDA
WITH
CROWNE /CAREHERE INC.
This Agreement made effective as of day of , 2011, between the City of Ocoee,
Florida, (hereinafter referred to as Ocoee), and the City of Belle Isle, Florida, (hereinafter referred to
as BeIIe Isle),
WITNESSETH:
WHEREAS, Florida law permits local governments to enter into interlocal agreements regarding mutual
aid, land use, zoning and planning and other items of mutual concern, and
WHEREAS, Ocoee has opened an on -site Employee Health Center, located at 17 Oakland Avenue,
Ocoee, Florida 34761, (hereinafter referred to as the Ocoee Center), and provides occupational services,
including pre - employment, annual physicals and other medical services through a third party
administrator CareHere, (hereinafter referred to as the Medical Services Provider),
WHEREAS, BeIIe Isle has a requirement to meet FDLE (Florida Department of Law Enforcement)
requirements for pre - employment physicals and has an on -going need for annual physicals and other
medical services for police officer applicants and full time employees,
WHEREAS, Ocoee desires to share the services of the Medical Services Provider with Belle Isle in
allowing qualified applicants and full time police officer personnel from BeIIe Isle this efficient and cost
effective manner of receiving medical services, and in accordance with the Contractual arrangements
between Ocoee and the Medical Services Provider, under the following conditions:
1. Belle Isle shall receive and pay a certain cost for medical services as listed and described on
Appendix "A ", as attached. Should the cost of the tests increase for Ocoee during the Term of this
Agreement, those increased costs shall be identified and passed through to BeIIe Isle,
2. CareHere shall create a reporting mechanism and monthly invoices for BeIIe Isle, extract the
Administrative fee as described on Appendix A and create a credit system on a monthly basis for Ocoee,
3. The initial term of this Agreement shall be for a period of twelve (12) months, beginning the
effective date above, and after the parties all approve this Agreement. Either party may terminate this
Agreement by providing thirty (30) days' notice, in writing, to the non - canceling party. Upon completion
of the initial term, this Agreement shall automatically renew for additional one (1) year periods unless
thirty (30) days written notice of cancelation is provided to the non - cancelling party. Any modifications
to this Agreement are not valid or binding unless they are in writing and formally approved by both the
City of Ocoee and the City of Belle Isle.
