Based upon the requirement to provide sufficient generator power, the City—as owner— installed the necessary equipment subsequent to the May date. The original estimate to complete the work including diagnostics on the first generator was 87 days or August 15, 1994. This was established in Change Order Number 23 as allowed in the emergency Commissioner agenda item on August 23, 1994. After discussions with the Architect and the Contractor, the dates for the construction to be sufficiently complete for the City's occupancy and for the commencement of warranties were agreed upon as follows: (1) Police Department, July 25, 1994; (2) City Hall including all site work, August 1, 1994. The appropriate certificates have been signed by the Architect and Contractor. They are the standard AIA document C704. Staff recommends that these certificates (Police Department and City Hall including all Site Work) be signed and that Change Order expenditures be confirmed. Action Requested The Mayor and Board of City Commissioners (1) confirmed Change Order Numbers 17 through 24; (2) approve the certification of Substantial Completion for the Police Department as July 25, 1994, and City Hall including all Site Work as August 1, 1994; and (3) authorize the Mayor and City Clerk to execute. GRGV Punchlist notes: Item #1 For all ductwork exterior of building, the Contractor shall provide Type D-3 and E-1 jackets, insulation type RR at 2" thick (Spec. Section 15560-8, 3.01). The lining shall be for three elbows from unit (PU-1). This requirement is greater than the external duct runs and shall be incorporated into the construction. Item #2 All undercuts are indicated on the mechanical sheets by an arrow and the abbreviations U.C. If the Contractor was unfamiliar with this notation he should have inquired about its meaning, rather than ignored its existence. Item #3 Sheet M-1, HVAC General note #6, indicates that there is a concrete pad for all mechanical equipment. Also note that condensate drain trap detail on Sheet M-4 indicates that the piping shall slope a minimum of 1/4" per foot. To attain this, the unit must be raised (4" pad will cover a 16' run of condensate piping). Not only does this apply to the Police Station, but the City Hall building also. Hsu and Associates, on September 12, 1994 we observed the constructed site with respect to our comments dated July 29, 1994 and have the following comments: Comments from July 29, 1994 that have not been addressed: 1. The June 23, 1994 report prepared by L.J. Nodarse stated "any areas needing repair have been pointed out to the contractor for repair before paving." However, the report does not indicate that the base was repaired and inspected before paving. This should be addressed by L.J. Nodarse. 2. The pavement and base should be cored to check for thickness. The pavement density should be tested. This should be addressed by L.J. Nodarg. 3. Comment deleted. (City Engineering Department has approved what we constructed.) 4. There are some bad paved areas on the driveway entrance to the parking area to the City Hall. The Contractor has stated in their September 9, 1994 letter to Buddy Peeler (herein referenced as the Contractor's letter) that the pavement that was not repaired was damaged by the City's landscaping operations. The Construction plans called for a header curb to be constructed from the end of the retaining va1l to the curb at the drive. Instead of the header curb construction, the contractor could place concrete blocks at the end of the retaining wall. This masonry work should maintain the existing height of the wall. Huber action reouired. 12. New driveway into the fire station site has no expansion joints. The driveway is not acceptable. The Contractor did not submit to the Engineer a Jointing plan for approval as required by the construction plans. Nor did the Contractor request PEC to review the formed driveway before the pour. We agree with the contractor that the driveway does not exhibit unusual stress cracking or other irregularaties now. Hoever, the driveway could develop stress cracking and irregularities at a later date. Huber action reouired. 13. Contractor shall submit as-built on the installed stormsewer system and stormwater pond. Combination lock on both East and West side door inoperable Need locks on doors leading to mens/womens shower areas Door to ladies shower room does not close properly. Shower rods never installed in either shower area. CABINET: New storage cabinet in Records Dept . was destroyed by a subcontractor who stepped on top of it and crushed it . Additionally, black scuff marks along wall where subcontractor climbed. Cracked tiles in shower stalls where screws were drilled. Records window inoperable/defective . Replace completely. Humidity problem throughout building, especially in Sergeant' s supply room. Building is very damp in the mornings . CEILING PANELS : Cracked panel in Sgt . Goclon' s Office Panel missing in Drug Locker Evidence Room: stains in several panels from leak in A/C ducts (leak repaired) Training Room: stains in several panels Weight Room: stains in panels Hallway: stains in panels Electrical plate cover missing in Evidence Room. Lengthwise crack in Records Room - above the shredder. Air Returns (2) have rust spots in Breakrocm. Cracked window tiles in Detective Tom' s office . Closure arm on hallway entrance door never installed. Need gutters .
